What To Bring:
A current referral from your referring doctor
All diagnostic investigations (X-rays, CT scans, MRI scans) relevant to your condition
A current list of your medications, particularly any blood thinners such as Aspirin, Plavix, Asasantin, Plaquenil, Warfarin, Apixaban (Eliquis), Fish oil, Tumeric
On the day:
Please arrive 5 minutes before the scheduled appointment time to allow time to fill in the acquaintance form
